Does the birth control pill cause weight gain?

Last reviewed July 6, 2026 by Dr. Sapna Jadhav, General Physician. Sources from ACOG, NHS, Mayo Clinic, CDC, NICE, NIH, Cochrane, and peer-reviewed journals.

Bottom lineLarge reviews, including a Cochrane analysis of 49 trials, find no evidence that the combined pill causes significant weight gain; temporary fluid retention in the first months is possible, and the contraceptive injection (Depo-Provera) is the hormonal method most clearly linked to genuine weight gain.

The short answer from the evidence is no - the combined birth control pill does not cause significant weight gain for most women.

What the research shows

A large Cochrane review pooling 49 trials found no evidence that combined hormonal contraceptives cause meaningful weight change compared with placebo or no method. On average, pill users gain no more weight over a year than women not using hormonal contraception. Weight naturally drifts upward slightly for most adults year over year, and the pill often gets blamed for that background trend.

Why the myth persists

The exception worth knowing

The contraceptive injection (Depo-Provera) is the hormonal method with the strongest evidence for genuine weight gain in some users. If weight is a concern, that distinction matters when choosing a method.

What to do if you notice weight gain

Track your weight, appetite, and bloating for 2-3 cycles after starting a new pill. If a clear pattern emerges, a different formulation or a non-hormonal method may suit you better - talk to your clinician rather than stopping abruptly.
