Does high prolactin make it harder to get pregnant?
Bottom lineYes - high prolactin makes conception harder mainly by suppressing GnRH so ovulation does not happen, and at milder levels of 20 to 50 ng/mL by shortening the luteal phase while cycles still look roughly normal. It is one of the more treatable causes of infertility: cabergoline or bromocriptine normalize prolactin in about 80 to 90% of cases, and ovulation and fertility usually return within weeks to a few months. If you have a prolactinoma and want to conceive, plan it with an endocrinologist first, since treatment is normally paused once pregnancy is confirmed.
Yes, mainly by stopping ovulation - but it is one of the more treatable causes of infertility, and fertility usually returns once prolactin is brought back to normal.
Why it affects fertility
High prolactin suppresses the GnRH signal that drives ovulation. Without an LH surge, no egg is released, and pregnancy is not possible that cycle.
At milder elevations of 20 to 50 ng/mL, you may still ovulate but have a short luteal phase - fewer than about 10 days between ovulation and your period. This is the version that catches people out, because cycles look roughly normal while conception quietly gets harder.
At higher levels, ovulation stops and periods usually stop with it.
The treatable part
Dopamine agonists such as cabergoline or bromocriptine normalize prolactin in roughly 80 to 90% of cases. Ovulation and periods typically return within weeks to a few months, and with them the ability to conceive. See how high prolactin is treated.
Worth knowing before you try
If you have a prolactinoma and want to conceive, plan it with an endocrinologist first. Treatment is usually paused once pregnancy is confirmed, and larger tumors need monitoring through pregnancy - all of which is much easier to arrange in advance than to react to.
When to get tested
If you have been trying for a year (or six months if you are over 35), or if your cycles are irregular, prolactin belongs on the blood panel alongside thyroid. Full detail in prolactin in women.
Femora's cycle tracking helps you spot a short luteal phase or missed ovulation before it becomes a year of trying.
